Dose reference #

Start 25 mg/day, increasing gradually toward 100 mg over two weeks. Later increases are gradual; adult maximum 250 mg/day. Early divided doses with meals improve tolerability.

Contraindications & important cautions #

Avoid MAOIs and use during acute recovery after myocardial infarction. Seizures, arrhythmia and serotonin toxicity are important concerns.

Adverse effects #

Anticholinergic and sexual effects, orthostasis, cardiac effects and seizure-related concerns require review.

Interactions #

Assess serotonergic combinations and metabolic inhibition; combining with SSRIs is not an automatic escalation strategy.
