Indications & formulation #

Dose reference #

Start 12.5 mg once or twice daily. The selected label permits 25–50 mg/day increments if tolerated toward 300–450 mg/day by two weeks; maximum 900 mg/day.

Contraindications & important cautions #

Severe neutropenia, myocarditis, seizures and gastrointestinal hypomotility require active monitoring. Baseline ANC and local monitoring requirements apply.

Interactions #

Smoking changes, infection and CYP1A2 modifiers can change exposure; treatment interruption changes restarting requirements.

Monitoring #

Use the applicable blood-monitoring and local clozapine protocols. Ask explicitly about bowel function, infection symptoms, cardiac symptoms and interruptions.

Special populations #

Consider reductions in significant renal/hepatic impairment or poor CYP2D6 metabolism; age, infection and smoking changes affect tolerability/exposure.

Discontinuation, restarting & counseling #

Interrupted treatment requires a reduced restart dose under the current label; never resume the previous full dose automatically.
