Indications & formulation #
Major depressive disorder in adults. This reference covers the selected formulation; other indications and products need their own evidence.
Dose reference #
Adult educational reference · Oral capsule · United States. The clinician selects treatment after assessment; this page does not calculate or change a prescription.
Start 20 mg each morning; reassess over several weeks before increasing. The selected adult MDD label limits the total to 80 mg/day.
Contraindications & important cautions #
Avoid MAOI combinations; allow at least five weeks after stopping fluoxetine before an MAOI. Pimozide and thioridazine are contraindicated combinations.
Selected important cautions; consult the full product information for all contraindications and warnings.
Adverse effects #
Activation, gastrointestinal and sexual effects; consider hyponatraemia, bleeding and serotonin toxicity in the appropriate context.
Interactions #
CYP2D6 inhibition and prolonged elimination matter when reviewing co-medication and switching. MAOI timing is product-specific.
Monitoring #
Review mood activation, target symptoms, function, adherence and adverse effects; assess sodium or other tests when clinically indicated.
Special populations #
Hepatic impairment and older age may require lower or less frequent dosing. Long elimination complicates switches.
Adult scope. Pregnancy, breastfeeding and pediatric use require the exact product’s population-specific guidance and individual assessment.
Discontinuation, restarting & counseling #
Review withdrawal and relapse even with its long half-life; interactions persist after the final dose.
Condition-specific prescribing #
Additional indication-specific records retain their own formulation and source context.
Fluoxetine
Adult bulimia nervosa; adjunct to comprehensive care
Adults · Conventional tablet/capsule; not weekly delayed-release product · Oral · US fluoxetine label; not a statement of Indian indication approval
- Starting dose
- Label-recommended dose is 60 mg each morning; some patients require gradual titration to it.
- Titration
- Individualize gradual introduction for tolerability; the cited label does not specify a universal incremental schedule for bulimia.
- Treatment range / target
- 60 mg/day is the evidence-supported target.
- Maximum / limits
- Doses above 60 mg/day have not been systematically studied for bulimia; do not import the depression maximum.
- Trial & duration
- Reassess acute response and periodically review continued benefit during maintenance.
- Monitoring
- Binge/purge frequency, activation, suicidality, sodium when indicated, bleeding and interactions.
- Population & organ-function adjustments
- Use a lower or less frequent dose in hepatic impairment and selected medically complex patients.
- Important cautions
- MAOIs and interacting serotonergic medicines; long persistence matters when switching. Assess electrolyte/cardiac vulnerability from purging.
