Formulation / routeOral tablet
Selected adult indicationAcute bipolar mania in adults; schizophrenia has a different dose discussion
MechanismDopamine D2 and serotonin 5-HT2 receptor antagonism.

Indications & formulation #

Acute bipolar mania in adults; schizophrenia has a different dose discussion. This reference covers the selected formulation; other indications and products need their own evidence.

Dose reference #

Adult educational reference · Oral tablet · United States. The clinician selects treatment after assessment; this page does not calculate or change a prescription.

Start 2–3 mg/day; increases of 1 mg no sooner than 24 hours if tolerated. Studied adult mania range: 1–6 mg/day.

Contraindications & important cautions #

Hypersensitivity to risperidone/paliperidone is a contraindication. Assess prolactin, movement symptoms, orthostasis and metabolic effects.

Selected important cautions; consult the full product information for all contraindications and warnings.

Adverse effects #

Movement symptoms, prolactin effects, orthostasis, sedation and metabolic effects require review.

Interactions #

CYP2D6 and other exposure modifiers matter. Oral and long-acting products are not interchangeable regimens.

Monitoring #

Review movements, prolactin-related symptoms, metabolic status, blood pressure and renal/hepatic context.

Special populations #

Severe renal/hepatic impairment: start 0.5 mg twice daily and titrate slowly. Dementia-related psychosis carries excess mortality risk.

Adult scope. Pregnancy, breastfeeding and pediatric use require the exact product’s population-specific guidance and individual assessment.

Discontinuation, restarting & counseling #

Review relapse risk and reduce under supervision; oral and depot regimens are not interchangeable.

Condition-specific prescribing #

Additional indication-specific records retain their own formulation and source context.

Adult schizophrenia: selected oral label

Adults · Immediate-release tablets · Oral · United States product labeling

Starting dose
2 mg/day, once or in divided doses.
Titration
Increase by 1–2 mg/day at intervals of at least 24 hours; slower titration may be appropriate.
Treatment range / target
Label target 4–8 mg/day; individualize to tolerability and response.
Maximum / limits
Label studied range extends to 16 mg/day. Doses above 6 mg/day with twice-daily dosing were not more efficacious and caused more EPS; this is not a routine high-dose target.
Trial & duration
Use an explicit adequate-dose trial and then reassess.
Monitoring
EPS, akathisia, prolactin symptoms, orthostasis and metabolic change.
Population & organ-function adjustments
Severe renal/hepatic impairment: start 0.5 mg twice daily; increases above 1.5 mg twice daily at intervals of at least 1 week.
Important cautions
Review CYP2D6 inhibitors, carbamazepine and cardiovascular risk; distinguish akathisia from agitation.
