| Version | Finalized adult English ITQ; 12 symptom items plus six functional-impairment ratings; developer source accessed October 2026 |
| Population | Adults aged 18 years and older |
| Respondent | Adult self-report |
| Recall period | PTSD and impairment: past month; DSO symptom items: typical ways of feeling, thinking and relating |
| Purpose | Record ICD-11 trauma symptom dimensions and source screening-rule patterns for clinician review |
Please identify the experience that troubles you most and answer the questions in relation to this experience. Brief description of the experience: discuss this with your clinician and record it in the clinical record outside this web questionnaire. Select when the experience occurred. Below are a number of problems that people sometimes report in response to traumatic or stressful life events. Please read each item carefully, then select one of the numbers to indicate how much you have been bothered by that problem in the past month. For the C1–C6 statements, think about how true each statement is of you: ways you typically feel, ways you typically think about yourself and ways you typically relate to others. C7–C9 ask about associated impairment during the past month.

Interpretation & limitations
- PTSD and DSO dimensional scores separately sum the six symptom items in each section, each 0–24. The six symptom-cluster scores each sum two items, each 0–8. Impairment ratings are assessed separately and do not enter these symptom totals. No overall sum cutoff is used.
- The source pattern requires at least one rating of 2 or greater in each relevant symptom pair and at least one associated impairment rating of 2 or greater. PTSD symptom clusters and impairment are required for either PTSD-related pattern; DSO alone does not satisfy either complete pattern.
- When both PTSD and DSO patterns are met, report the combined pattern for clinician review instead of simultaneously reporting two diagnoses. This questionnaire does not generate a diagnosis. Trauma exposure, differential assessment and clinical judgment remain necessary.
- All 18 symptom and impairment ratings are required for a complete report. The event-timing question is contextual and does not contribute to scoring.
- Scores and screening-rule patterns require clinical evaluation. No automatic diagnosis, medication change or declaration of safety.
