| Purpose | Suicidal Behaviors Questionnaire-Revised (SBQ-R) — administration and scoring reference. |
Administration
Use the original SBQ-R response options and their different time frames. Discuss any reported suicidal behavior or current concern regardless of the aggregate score.
Scoring method
Four differently anchored item responses scored per source; no declaration of safety.
Interpretation & limitations
- Population-specific screening cutoffs are not universal risk estimates. Interpret the pattern of responses with a direct current assessment.
- Interpret the findings alongside the clinical interview, functioning, history and relevant physical assessment.
- A low total cannot establish safety, and a total must not determine discharge, observation level or treatment automatically.
- Use the original instrument and manual for item wording, rating anchors, missing-data rules and any normative conversions. This reference does not establish a diagnosis or select treatment.
