| Purpose | Wender Utah Rating Scale (WURS) — administration and scoring reference. |
Administration
Ask adults to rate childhood experiences using the selected WURS form. Obtain collateral childhood records when feasible.
Scoring method
Retrospective childhood-symptom ratings; original and abbreviated editions differ.
Interpretation & limitations
- A retrospective score supports exploration of childhood symptoms and cannot independently establish childhood onset.
- Interpret the findings alongside the clinical interview, functioning, history and relevant physical assessment.
- Do not sum all items from the full form and compare that number with WURS-25 thresholds. Recall and current mood may affect answers.
- Use the original instrument and manual for item wording, rating anchors, missing-data rules and any normative conversions. This reference does not establish a diagnosis or select treatment.
