Sleep-diary summary

Descriptive calculations for clinician review. This is an original working aid, not a standardized questionnaire.

Use a date and time for each event, including the following day for overnight sleep. Enter zero when no latency or awakenings occurred; leave unused periods entirely blank. Record estimates after waking. Daytime sleep periods may also be entered, once each.

Sleep period 1
Calculation method and limitations

Total sleep time = final awakening minus time trying to sleep, minus sleep-onset latency and time awake after sleep onset. Time in bed = out-of-bed time minus into-bed time. Sleep efficiency = total sleep time / time in bed × 100. Pre-sleep time in bed and time after the final awakening are included in this denominator. Summary efficiency uses summed sleep time / summed time in bed; it is not the unweighted mean of nightly percentages.

Dates are treated as local wall-clock times; this tool does not adjust for daylight-saving transitions or travel. Diary estimates are not objective sleep measurements. No sleep-restriction schedule, diagnosis or medication advice is generated.
